(If there is connected equipment damage, do not send in surge suppressor separately. Refer to section on “Connected Equipment Claims” below.)

Include inside the shipping box*:
• Your name
• A physical address (where new product can be shipped)
• A phone number where you can be reached
• A brief description of the problem with the product

* If multiple products are being returned, include the above information in each shipping box.


Connected Equipment Claims
1. Call 1-800-877-1174, ext. 473
2. Retain surge suppressor to submit with claim packet. Do not submit separately.

ServiceTracker Claims
1. Mail the following items:
 

(1) ServiceTracker Claim form (rev date 6/14/02)**,
(2) A copy of your service call work order, and
(3) The connected surge suppressor (only if damaged)
